  • Patient Last Name
  • Desired Payment Amount
  • Click Next

Step 2

Must enter the following information:

  • Enter your Credit Card Number
  • Enter the Credit Card Expiration Date
  • Enter CVC / Security Code
  • Click Next

Step 3

Must enter the following information:

  • Enter the First & Last Name of the Credit Card Owner
  • Select the Billing Country of the Card (United States Default)
  • Enter the Billing Street Address of Your Credit Card
  • Enter the Rest of the Billing Address after entering Street Address
  • Click Submit

Step 4

Must Click Submit Payment

  • Click Submit Payment

Step 5

Confirmation

  • Green “Payment received” is a confirmation
  • You will also receive an email confirmation of payment
